| Welcome to our office. Please arrive 15 minutes before your scheduled appointment. We require that you bring your insurance cards and a picture identification. Make sure that you have any needed referrals. A list of your currant medications would be helpful as well. If you have any reports from previous treatments that could be helpful, please bring those as well. Make sure to allow yourself adequate time; initial visits are more comprehensive. All forms can be downloaded from our website and filled out in advance of your visit. We look forward to helping you soon. |
